Birth Certificate

A copy of a certified birth certificate can be requested by printing and completing the appropriate application form found in the link on the bottom of this page.  Forms can be presented in person to the Tax Payer Service Center, located on the first floor of the Courthouse in Benson. 

 

If you cannot present in person to request a copy, your signature on the application must be notarized and a check must be included with your request. Mailed requests should be sent to:

Benson, MN  56215.

 

The fee to obtain a copy of your birth certificate is $26 payable to Swift County Treasurer. Please note: requests will not be processed until payment has been received.
